If you incur an injury at work you must fill out an injury report and you and your building administrator must sign it. Then forward your report to Human Resources.

If medical attention is not required you still need to fill out an injury report, mark at the top "For Information Only" and forward it to Human Resources.

If medical attention is necessary - Building secretary will advise HR. HR will call Ascension Michigan at Work
